The undersigned parent(s) wish their child,

to participate in the Cincinnati Christian Schools, Inc.
The undersigned parent(s) understand and acknowledge that no transportation shall be provided relating to said function noted above. The undersigned parent(s) agree that their child may be provided transportation by another parent volunteer or student to the said function noted above. The undersigned further knowingly and voluntarily release and waive, and further agree to indemnify and hold harmless Cincinnati Christian Schools, Inc., the school board members of Cincinnati Christian School, Inc., Cincinnati Christian Schools Administration, agents, employees, volunteers, representative successors or assigns thereof individually and in any capacity or relationship with or for any other, for or on account of any and all claims, including but not limited to, bodily injuries, and pain and suffering, which arises or may arise from the transportation of their child to and from the function noted above.
