Immunizations: The Ohio Department of Health allows fifteen days after a student enters a school for obtaining the necessary information to complete the school health record. All students need to have a copy of their immunization record and kindergarten students also need to have the Ohio Physical Form completed by their physician. Immunization requirements can be obtained from the nurse’s office and are also available here.
Dental: The dental form for kindergarten students is optional. This form can be found on this web page under the title “Dental Form”.
Medication at school: Students may not carry or administer their own medications. Written permission from both a parent and physician is required for both non-prescription medications and prescription medication. All medicine must come in their original container. Forms for both non-prescription and prescription medications are available from the nurse or on this web page under the title “Prescription Medication Form” and “Non-Prescription Medication Form”. Students may carry their own asthma inhaler if they have a written order from their physician and a signed form from their parent on file with the school nurse.
